in orange glass bottles of 100 g; in a box 1 bottle (a measure with a scale is attached to the package).

Description of the dosage form

Suspension orange color with an orange smell and sweet taste, with a slightly perceptible burning aftertaste. There may be a separation into a liquid layer and a precipitate, which, after mixing, constitute a homogeneous suspension.

pharmachologic effect

Antipyretic, analgesic, anti-inflammatory. It blocks arachidonic acid cyclooxygenase and reduces the synthesis of PG. The antipyretic result is due to a decrease in the concentration of PG in the cerebrospinal fluid, a decrease in the excitation of the thermoregulation center, the result of which is the normalization of body temperature. The analgesic effect is associated with a decrease in the production of PG classes E, F, I, biogenic amines, a change in the sensitivity of nociceptors and the prevention of the development of hyperalgesia. The analgesic effect is most pronounced with inflammatory pain. The anti-inflammatory result is due to a decrease in the secretion of inflammatory mediators and a decrease in the activity of the exudative and proliferative phases of the inflammatory process. The antipyretic and analgesic effect is expressed earlier and at lower doses than the anti-inflammatory effect that occurs on the 5-7th day of treatment.

Pharmacokinetics

After oral administration, more than 80% is absorbed from the gastrointestinal tract. Cmax in plasma is achieved when taken before meals - after 45 minutes, when taken after meals after 1.5-2.5 hours. Protein binding - 90%. It slowly penetrates into the joint cavity, however, in the synovial fluid it creates concentrations greater than in the blood plasma (Cmax in the synovial fluid is reached after 2-3 hours). It is metabolized usually in the liver. It undergoes pre- and post-systemic metabolism. After absorption, about 60% of the pharmacologically inactive R-form is slowly transformed into active S-shape. Excreted by the kidneys (60-90% in the form of metabolites and products of their combination with glucuronic acid, to a lesser extent - with bile, unchanged - no more than 1%). It has a two-phase elimination kinetics with T1 / 2 2-2.5 hours, after administration in a single dose it is completely eliminated within 24 hours. The antipyretic effect of Ibufen develops after 30 minutes and lasts 6-8 hours.

Indications for Ibufen

As an antipyretic: with colds, acute respiratory viral infections, influenza, tonsillitis (pharyngitis), childhood infections accompanied by fever, post-vaccination reactions.

As a pain reliever: with toothache, painful teething, headache, migraine, neuralgia, pain in muscles, joints, injuries and burns.

Contraindications

Hypersensitivity (including to acetylsalicylic acid or other NSAIDs), peptic ulcer of the stomach and duodenum, severe insufficiency of the liver, kidneys, of cardio-vascular system, arterial hypertension, hemophilia, hypocoagulation, hemorrhagic diathesis, deficiency of glucose? 6-phosphate dehydrogenase, bronchospastic reactions after the use of acetylsalicylic acid or other NSAIDs ("aspirin asthma"), Quincke's edema, nasal polyps, hearing loss, infancy (up to 6 months, with body weight - below 7 kg).

Side effects

From the side of the central nervous system: headache, dizziness, sleep disturbance, anxiety, drowsiness, depression, agitation, visual impairment (reversible toxic amblyopia, blurred vision or double vision).

From the side of the hematopoietic organs: heart failure, tachycardia, increased blood pressure; anemia, thrombocytopenia, agranulocytosis, leukopenia.

From the digestive tract: nausea, vomiting, loss of appetite, heartburn, abdominal pain, diarrhea, constipation, flatulence, abnormal liver function, peptic ulcers, gastric bleeding.

From the urinary system: acute renal failure, allergic nephritis, nephrotic syndrome (edema), polyuria, cystitis.

Allergic reactions: itching, rash, bronchospastic syndrome, allergic rhinitis, angioedema, Steven-Johnson syndrome, Lyell's syndrome.

Interaction

It should not be combined with other NSAIDs (acetylsalicylic acid reduces the anti-inflammatory effect and increases side effects). When taken simultaneously with diuretics, the diuretic effect decreases and the risk of developing renal failure increases. Weakens the impact antihypertensive drugs including ACE inhibitors (simultaneously reduces their excretion by the kidneys), beta-adrenergic agents, thiazides. Enhances the effect of oral hypoglycemic agents (especially sulfonylurea derivatives) and insulin, indirect anticoagulants, antiplatelet agents, fibrinolytics (increases the risk of hemorrhagic complications), the toxic effects of methotrexate and lithium preparations, increases the concentration of digoxin in the blood.

Inducers of microsomal oxidation (phenytoin, ethanol, barbiturates, zixorin, rifampicin, phenylbutazone, tricyclic antidepressants) increase the risk of developing severe hepatotoxic complications (increase the production of hydroxylated active metabolites), inhibitors of microsomal oxidation decrease. Caffeine enhances pain relief.

Dosage and administration

inside, after meal. The average single dosage is 5–10 mg/kg of body weight 3–4 times a day. Children aged 6 months - 1 year (7-9 kg)- 2.5 ml (50 mg) 3 times a day, the highest daily dosage is 7.5 ml (150 mg). 1–3 years (10–15 kg)- 2.5 ml (50 mg) 3-4 times a day, the largest daily dosage is 7.5-10 ml (150-200 mg). 3–6 years (16–20 kg)- 5 ml (100 mg) 3 times a day, the largest daily dosage is 15 ml (300 mg). 6–9 years old (21–30 kg)- 5 ml (100 mg) 4 times a day, the largest daily dosage is 20 ml (400 mg). 9–12 years old (31–41 kg)- 10 ml (200 mg) 3 times a day, the largest daily dosage is 30 ml (600 mg). Over 12 years old (more than 41 kg)- 10 ml (200 mg) 4 times a day, the highest daily dosage is 40 ml (800 mg). The dose may be repeated every 6-8 hours. Do not exceed the maximum daily dosage. For children from 6 months to 1 year, the drug is prescribed on the recommendation of a doctor.

Overdose

Symptoms: abdominal pain, nausea, vomiting, lethargy, headache, tinnitus, depression, drowsiness, metabolic acidosis, hemorrhagic diathesis, decreased blood pressure, acute renal failure, abnormal liver function, tachycardia, bradycardia, atrial fibrillation; convulsions, apnea and coma (especially characteristic of children under 5 years of age).

Treatment: gastric lavage, appointment activated carbon, alkaline drinking, symptomatic therapy (correction of acid-base balance, blood pressure).

Precautionary measures

Caution should be taken with cirrhosis of the liver with portal hypertension, liver and / or kidney failure, heart failure, nephrotic syndrome, hyperbilirubinemia, peptic ulcer of the stomach and duodenum (in history), gastritis, enteritis, colitis, blood diseases (leukopenia, anemia) , pregnancy (II-III trimesters), during breastfeeding. In patients with bronchial asthma or other diseases that occur with bronchospasm, an increased risk of developing bronchospasm is likely.

During long-term treatment, it is necessary to control the picture of peripheral blood and the functional state of the liver and kidneys. When symptoms of gastropathy appear, careful monitoring is indicated (conducting esophagogastroduodenoscopy, a blood test with the determination of hemoglobin, hematocrit, fecal occult blood analysis).

In the absence of an antipyretic effect for 2 days and an analgesic effect for 3 days, you should consult a doctor. In case of side effects, stop taking the medicine and consult a doctor.

Description medicinal product IBUFEN D based on officially approved instructions for use of the drug and made in 2012. Date of update: 12/28/2011

pharmachologic effect

NSAIDs, a derivative of propionic acid. It has antipyretic, analgesic and anti-inflammatory effects.

The mechanism of action of ibuprofen is primarily due to the suppression of prostaglandin biosynthesis by reducing the activity of cyclooxygenase (COX), an enzyme that regulates the conversion of arachidonic acid into prostaglandins, prostacyclin and thromboxane. At the same time, as a result of irreversible inhibition of the cyclooxygenase pathway of arachidonic acid metabolism, the formation of prostaglandins decreases. A decrease in the concentration of prostaglandins at the site of inflammation is accompanied by a decrease in the formation of bradykinin, endogenous pyrogens, other biologically active substances, oxygen radicals and NO. All this leads to a decrease in the activity of the inflammatory process (anti-inflammatory effect of ibuprofen) and is accompanied by a decrease in pain reception (analgesic effect).

A decrease in the concentration of prostaglandins in the cerebrospinal fluid leads to the normalization of body temperature (antipyretic effect). The decrease in temperature begins 30 minutes after ingestion, its maximum effect is manifested after 3 hours.

The use of Ibufen D suspension helps to reduce the manifestations of symptoms such as fever, pain and swelling, by acting at the site of their occurrence.

Pharmacokinetics

Suction

After oral administration, more than 80% of ibuprofen is absorbed from the gastrointestinal tract, reaching Cmax in blood plasma after 1 hour. Eating slows down the absorption of ibuprofen, but does not reduce its bioavailability. The time to reach C max when taken with food is 30-60 minutes longer than when taken on an empty stomach and is 1.5-3 hours.

When taking ibuprofen in the form of a suspension in children at a dose of 10 mg / kg, its Cmax is 55 μg / ml.

Distribution

Ibuprofen binds to plasma proteins (mainly albumin) by more than 99%. A high degree of protein binding limits the rate of penetration of ibuprofen into the synovial and cerebrospinal fluid.

V d of ibuprofen in adults is about 0.12 l / kg of body weight, but in children with fever under 11 years of age, V d is about 0.2 l / kg of body weight.

Ibuprofen slowly penetrates the synovial fluid, keeping it long time therapeutic concentration. Does not accumulate in the body.

Metabolism

Ibuprofen is biotransformed mainly in the liver.

breeding

70-90% of the dose taken is excreted in the urine in the form of metabolites and products of their conjugation with glucuronic acid. The remainder of the dose is excreted in the feces unchanged and also in the form of metabolites. T 1/2 - 2-2.5 hours

Dosing regimen

The drug is taken orally after meals, with plenty of water. Before use, the vial should be shaken until a homogeneous suspension is obtained. For accurate dosing of the suspension, a dispenser (spoon or syringe) is attached to the vial.

The dose is set depending on the age and body weight of the child. When calculating the dose, it should be taken into account that 5 ml of the suspension contains 100 mg of ibuprofen.

A single dose of Ibufen D is 5-10 mg/kg of the child's body weight 3-4 times/day. The maximum daily dose of the suspension is 20-30 mg/kg of body weight, not more. The drug is prescribed in single doses according to the scheme given in the table below:

Taking the drug in a single dose can be repeated every 6-8 hours with an interval of at least 4 hours between doses in the next dose.

Children up to 6 months you can use the drug after consulting a doctor in accordance with his recommendations. Do not exceed the recommended dose.

At post-vaccination reactions in children from 3 to 6 months the dosage regimen of the drug is determined by the doctor.

Duration of treatment - no more than 3 days as an antipyretic, no more than 5 days - as an anesthetic.

If the fever persists for more than 3 days, a doctor should be consulted.

Instructions for use dispenser in the form of a syringe

When an overdose exceeded 400 milligrams per kilogram of the child's weight, a situation is created that seriously threatens the life and health of the baby. In order to avoid serious consequences of the toxic effects of the drug on the body, you should seek help as soon as possible.

Overdose should be treated with rapid treatment by gastric lavage. Activated charcoal, alkaline drinking is prescribed. It is advisable to carry out symptomatic therapy.

    Drug Interactions

In order to avoid a decrease in the anti-inflammatory effect and an increase in side effects, one should refrain from taking Ibufen simultaneously with other drugs of the NVPS group.

You should not combine Ibufen with diuretics, as this can lead to a decrease in their effectiveness and increase the risk of kidney failure.

When combined with Ibufen, the hypotensive activity of vasodilators decreases.

The drug enhances the effectiveness of oral hypoglycemic agents, as well as insulin.

An increased risk of developing severe hepatoxic reactions can be expected when Ibufen is combined with inducers of microsomal oxidation (rifampicin, phenytoin, barbiturates, tricyclic antidepressants, phenylbutazone, and ethanol).

Absorption of Ibufen will be reduced under the influence of antacids.

Hypoprothrombinemia develops more often when Ibufen is combined with valproic acid, cefotetan, cefoperazone and other similar medicines.

The hematotoxicity of the drug Ibufen will manifest itself more strongly when exposed to drugs with myelotoxic action.

Combination with gold preparations and cyclosporins leads to an increase in the synthesis of prostaglandin in the kidney area under the influence of Ibufen, which serves as a development of an increase in the nephrotoxic effect.

Plasma concentrations of cyclosprin increase when combined with ibuprofen, increasing the risk of hepatoxicity.

The plasma concentration of ibuprofen will increase and its excretion will decrease when taken simultaneously with those drugs that are capable of blocking tubular secretion.

Ibufen enhances the action of the following drugs: antiplatelet agents, indirect anticoagulants, fibrinolytics, which is fraught with hemorrhagic complications.

Ibuprofen helps to increase the concentration of digoxin, lithium preparations and methotrexate in the blood.

Combining Ibufen with caffeine enhances pain relief.

    Additional instructions

With prolonged use of the drug, the functionality of the kidneys and liver, as well as the peripheral blood picture, should be monitored.

For some studies, the patient is asked to cancel the drug two days before they are carried out.

During the treatment period, drugs containing ethanol should not be taken.

In order to avoid unwanted side effects from the digestive tract, Ibufen is recommended to be taken in doses of the lowest effective and as short courses as possible.

Since the side effects of the drug Ibufen are often quite serious, you should, during treatment, engage in activities related to mechanisms and driving any vehicles as little as possible.

    Ibufen children's analogues

Analogues of the drug Ibufen, which can successfully replace the drug if necessary, can be considered medicines: Advil for children, Ibuprofen-Hemofarm, Nurofen for children, Ibuprofen. Each of them is available in suspension form.

    Ibufen for children price

The cost of the drug is not high and does not exceed 85 rubles.

    Ibufen children's reviews

Judging by the reviews, the drug Ibufen is very popular with parents of babies as an effective antipyretic and analgesic. Many praise not only the effectiveness of the drug, but also a convenient form of release, as well as simple dosing. According to the parents, there is no problem to give medicine to the baby with the help of dosing devices, as well as due to its pleasant aroma and taste.

There are a lot of reviews and each of them praises and recommends Ibufen to those who are not familiar with it yet. We present, perhaps, for clarity, some of them.

Nina: When my daughter, having come from the kindergarten, began to literally “burn”, I rushed to the first-aid kit at home. Antipyretic candles brought down the temperature a little and the thermometer recorded 39.5 degrees, but the effect was not long. Frightened by the further unfavorable development of events, I called the doctor. We were recommended Ibufen for children. I must say, he put the child on his feet in a jiffy. After the temperature was brought down, no cold symptoms followed. Virus. Now Ibufen has also settled in our medicine box.

Svetlana: As a toddler, our son was often sick and each time, the temperature rose quite high. I was always saved by Nurofen, until I was advised at the pharmacy for its absence, the drug Ibufen. It turned out that there is no difference between them, except for the cost, and it seemed to me that Ibufen helped faster and more efficiently. Now we are expecting a second baby and we are no longer afraid of colds and viruses. After all, we know about Ibufen.

Ibufen D is probably the most popular antipyretic for children, but Ibufen D is contraindicated in children under 6 months (body weight less than 7 kg).

Ibufen D - suspension for oral administration: as an antipyretic: for colds, acute respiratory viral infections, influenza, tonsillitis (pharyngitis), childhood infections accompanied by fever, post-vaccination reactions.

Ibufen D is a non-steroidal anti-inflammatory drug that also has analgesic and antipyretic effects. Refers to preparations containing a derivative of propionic acid ibuprofen.

    Pharmacotherapeutic group

Non-steroidal anti-inflammatory drugs (NSAIDs), propionic acid derivatives.

    ATC code: M 01 AE 01

Pharmacological properties of the drug Ibufen D

    Pharmacokinetics

When taken orally, more than 80% of ibuprofen is absorbed from gastrointestinal tract, reaching the maximum concentration in the blood plasma after 1 hour. Eating slows down the absorption of ibuprofen, but does not reduce its bioavailability. When taking ibuprofen in the form of a suspension in children at a dose of 10 mg / kg, its maximum concentration in the blood is 55 μg / ml. Ibuprofen when taken with food is absorbed more slowly: (t max) while 30-60 minutes longer than when taken on an empty stomach and is 1.5-3 hours.

Ibuprofen binds to plasma proteins by more than 99%. The major drug-binding proteins are albumins. The volume of distribution of ibuprofen in adults is about 0.12 l / kg of body weight, but in children with fever under 11 years of age it is higher and is about 0.2 l / kg of body weight. A high degree of protein binding limits the rate of penetration of ibuprofen into the synovial and cerebrospinal fluid. Therefore, ibuprofen slowly penetrates into the synovial fluid, maintaining a therapeutic concentration in it for a long time. The drug is biotransformed mainly in the liver. 70-90% of the dose taken is excreted in the urine in the form of metabolites and products of their conjugation with glucuronic acid.

The rest of the dose is excreted in the feces unchanged and also in the form of metabolites. The drug does not accumulate in the body.

    The half-life is 2 - 2.5 hours.

    Pharmacodynamics

Ibuprofen is a propionic acid derivative that has antipyretic, analgesic and anti-inflammatory effects.

The effect of lowering the temperature begins 30 minutes after ingestion, its maximum effect is manifested after 3 hours.

Ibufen D - suspension has antipyretic, analgesic, anti-inflammatory effect. Reduces the manifestation of symptoms such as fever, pain and swelling, by acting at the site of their occurrence.
